Palliative Care
Highland Primary Care Network Palliative Care Consultation Team directs funds towards helping palliative patients to remain in the community through the use of flexible community funds when no other sources of funds are available.
This funding also assists family physicians to care for palliative patients in the community and assists in linking physicians with specialists in Palliative Care.
Population Health
A Population Health Facilitator is assessing which health issues are most prevalent within the communities of Airdrie, Balzac, Beiseker, Carstairs, Cremona, Crossfield, Didsbury, Irricana, Kathyrn, Madden and the surrounding areas. This work is coordinated with the Alberta Health Services population health team in order to promote overall improved health of patients in the area. One current focus is the issue of high smoking rates among pregnant women in Airdrie which appears higher than any other community in or around Calgary.
Woman’s Health Screening - periodic health exam
Highland Primary Care Network is working with the Alberta Cancer Board to promote women’s health screening including cervical screening. A pilot project will use mailings (direct and indirect) to increase the number of women screened and to evaluate the economics of registered nursing in cervical screening. Pilot project resources will increase screening capacity in our area by training local nurses in cervical screening. Overall, the project will encourage women to have a general health examination at regular intervals.
