Complex Care 

The Highland Health Clinic – Complex Care Team provides an interdisciplinary approach to complex patient

care. The team includes a Family Physician, Registered Nurse, Pharmacist, Social Worker, and Dietitian. The team works together with the patient to help them manage their health. The care provided is carried out in close partnership with their family physician and the other health professionals involved in their care. 

Who we want to see (Can have one or more of the following):

People who:

  • Have one or more chronic medical condition
  • Are having difficulty managing
  • Need more intensive monitoring or care
  • Are having issues regarding medication management
  • Are on multiple medications
  • Have had repeat visits to physician office, ER, urgent care or hospital admissions

 

Patients can be referred to this service by any health care provider by:

  • Faxing a consult letter to 403 948-8627 OR
  • By leaving a detailed message on our 24 hr referral line at 403 948-6389.

Chronic Disease Management

Members of the complex care team also work in various physician offices within the PCN to optimize the health and well being of people living with chronic disease. The team members see patients with complex illness including, but not limited to, the following chronic diseases:

  • Diabetes
  • High cholesterol
  • Chronic Pain
  • Obesity
  • Congestive heart failure
  • Asthma/COPD
  • High Blood Pressure

The number of team members that work with a patient will depend on the medical condition(s). 
 
 

The Highland Health Clinic

The Highland Health Clinic offers a variety of programs under one roof. In addition to being the home of the complex care clinic, it houses a thriving family practice and offers a community location for patients to see the following providers:

  • Adult Asthma/COPD educator (includes education on smoking cessation and chronic cough)
  •  Behavioural Health Consultant
  • Dietitian
  • EENT
